For many, grief begins before a death occurs. Sacred Transitions exists as a resource for the hospice professionals of Colorado to aid families through the emotional burden of losing a loved one. Our program helps equip caretakers with the knowledge and tools to guide families through moments of transition with tenderness and tranquility, answer their questions and concerns, and help them embark on their journey to healing.
